FollowMyHealth provides user-friendly navigation to encourage individuals and their families to participate in their health care by taking a more active role in securely managing their well-being. Accessed via the Web or through a mobile app, FollowMyHelath is a more convenient way for patients to stay connected - providing patients with the ability to securely message physicians with questions regarding their health, view upcoming appointments, tests and lab results, and renew medications.

HOW TO SIGN UP FOR A FOLLOW MY HEALTH ACCOUNT:
Contact Parkview Medical Clinic to get connected. We will need your preferred email address. Once we send you an invite, you can set up your own account/log-in.
If you already have a Follow My Health account through another health care provider, you do not need to set up another account. Just contact us and we will send you an invite to connect to your existing account.

FREQUENTLY ASKED QUESTIONS
Q: Can I make an appointment online using Follow My Health?
A: Yes, you are able to make an appointment request using Follow My Health. However, we do request if you are making a same-day appointment request or sending an appointment request after hours for the next day, please call our mainline at 952-758-2535 to ensure we can accommodate your needs. You will receive a response back from us with appointment details or follow-up questions depending on availability.
Q: Can I get access to my child's record through Follow My Health?
A: Yes, you can get proxy access which allows you to have access to their record. This access automatically halts once your child turns 18. Contact Parkview Medical Clinic and ask for proxy access to your child's account.
Q: I forgot my username and/or password. What should I do?
A: Due to security reasons, Parkview Medical Clinic does not have access to this information. If you forget your password, you can click the "forgot password" link at Follow My Health. If you are still having issues and forgot your username, you will need to contact Parkview Medical Clinic to disconnect your account and send you a new invite.
Q: I want to access my spouse's account, can I get access so we have one family account?
A: Due to patient privacy legalities, we cannot accommodate this unless the other adult consents to this. In this event, the account would be set up similar to a proxy account.
Q: I am having issues seeing all of my information or the Follow My Health website seems to have issues.
A: Try logging in via a different internet browser or method.
Q: I recently had labs done but I do not see them in my Follow My Health Account.
A: Labs will not be sent to the Follow My Health patient portal until they have been reviewed and finalized in our system by the provider. If a provider is out or waiting on items for your health, they may not finalize these right away.
Q: I created a Follow My Health Account, but I do not see my information.
A: If Parkview Medical Clinic did not send you an invite, then you cannot connect to your account. Please call us and ask that we send you an invite so you can follow the steps to connect your account to your record. If Parkview Medical Clinic sent you an invite, you have connected but cannot see your information, please call us so we can try to assist you in troubleshooting. In this case, we may need to disconnect your account and send you a new invitation to connect.
